Provident Healthcare has been recognized by the National Committee for Quality Assurance (NCQA) as a Level III Patient Centered Medical Home Practice (PCMH). Provident is one of a handful of practices in the state of Colorado that have been given this prestigious recognition.
What is a Patient Centered Medical Home?
A practice under the Patient Centered Medical Home creates an environment that promotes the relationship between the patient and their provider, as well as the patient’s family when deem appropriate.
In this care model the physician-patient relationship replaces the episodic care based on illnesses and patient complaints with a coordinated care and a long-term healing relationship. The physician-led care team is responsible for providing all the patient’s health care needs and, when needed arranges for appropriate care with other qualified physicians.
What does it mean to be a Recognized by NCQA as a PCMH?
There are nine PPC® standards (listed below), including 10 must pass elements, which can result in one of three levels of recognition, including the use of electronic medical records. Provident has attained the highest level of recognition at Level III.
